If you believe that your content was removed or disabled by mistake or misidentification, you may submit a counter-notification pursuant to the DMCA. Your counter-notice must include the following information:

2. Identification of the material that has been removed or to which access has been disabled and the location at which the material appeared before removal.
3. A statement under penalty of perjury that you have a good faith belief the material was removed or disabled as a result of mistake or misidentification.
4. Your name, address, telephone number, and email address.
5. A statement that you consent to the jurisdiction of the federal court in your judicial district, and that you will accept service of process from the person who submitted the original DMCA notification.

3. Repeat Infringers

FindWayBiz.com reserves the right to terminate accounts or block access of any user who is found to be a repeat infringer.
